We can guide you through a 10-minute consultation process; additional time is billable in 15-minute increments, including phone calls. Our schedule is reserved for those who consent to treatment. *Advice is provided during appointments, and after your INFORMED CONSENT is signed.

TO PAY, please add your credit card to your Jane App account. Cash and checks are appreciated for in-person appointments. Your Specialist Copay is due by your appointment. For those without insurance, Direct-Pay is available for $150 per hour, billable in 15-minute increments. *MEDICAID Members DO NOT have a Copay.
NO SHOWS and/or cancellations with less than 24 hours' notice can be rescheduled by paying the non-refundable value of your Specialist Copay or Direct-Pay fee in advance.

CHECKLIST FOR SERVICES
STEP 1 YOU call Member Services to find out if you have Medical Nutrition Therapy MNT INSURANCE benefits. A referral is required.
STEP 2 YOU request a REFERRAL from your physician fax to {850.203.4429}. To optimize coverage, referral should specify the reason for MNT and include supportive diagnoses beyond a BMI, labs, and medical history.
STEP 3 WE submit your referral for PRIOR AUTHORIZATION and contact you once it’s approved or denied by your insurance company.
STEP 4 YOU sign INFORMED CONSENT via DocuSign, as required to schedule an appointment (check spam).
STEP 5 WE SCHEDULE in Eastern Time Zone. All appointment confirmations come with a telehealth link. Chrome is the optimal browser to access your JaneApp profile we initiate.
STEP 6 YOU accept JaneApp welcome prompt to set up a login, please add your credit card, and set your time zone. Please be prepared to complete the questionnaires before your appointment and take notes during our session.
TELEHEALTH services by telephone, data-based video calls, fax, email, and text messages. A video consult is required for telehealth. **DRIVING during your telehealth appointments is PROHIBITED and will be discontinued until you respect your appointment time.
The HIPAA-compliant SOFTWARE we use includes our electronic health record is JaneApp -for scheduling, appointment reminders, telehealth, notes/documents, and billing; the Spruce app; and DocuSign.
