INSURANCE COVERAGE
NUTRITION ABBEY provides Clinical Nutrition services via telehealth, across Florida. We can guide you through a brief consultation process, additional time is billable in 15-minute increments, including phone calls. Advice is provided during appointments, after the Informed Consent process. *GUARDIANS of the referred patient, please inform and provide validation.
PAYMENT options for services rendered include Direct-Pay, insurance payer with Specialist Copay, or you can submit to your insurance company for reimbursement following Direct-Pay. To pay, please add your credit card to your Jane App account, cash and checks are accepted for in-person appointments.
For INSURANCE COVERAGE, a referral from your physician is required, followed by prior authorization from your insurance company. If your insurance payer is not listed, call Member Services to determine if you can submit for reimbursement. NUTRITION ABBEY is contracted with the following insurance payers:
STEP 1
Call Member Services on the back of your insurance card. Find out if you have Medical Nutrition Therapy benefits.
STEP 2
If you have Medical Nutrition Therapy benefits, please request a referral from your physician. To optimize coverage, your referral should specify the reason for Medical Nutrition Therapy and include supportive diagnoses, beyond a BMI. Referrals can be faxed to 850.203.4429.
STEP 3
NUTRITION ABBEY and your insurance company will process your referral and prior authorization if you are eligible for benefits and have a qualifying diagnosis.
STEP 4
Once your insurance company approves your prior authorization, we will contact you. An Informed Consent for NUTRITION ABBEY services will be sent from DocuSign (check spam).
STEP 5
Sign Informed Consent via DocuSign for NUTRITION ABBEY clinical services.
STEP 6
Schedule with NUTRITION ABBEY!
