FAQ CHECKLIST FOR SERVICES

  • STEP 1‍ ‍YOU call Member Services to find out if you have MNT benefits and qualifying diagnoses.  
    INSURANCE Payers contracted for Medical Nutrition Therapy MNT services: Capital Health Plan; Gold Kidney of Florida; FL Community Care; Aetna; Simply Healthcare: FL Medicaid, Florida Healthy Kids & Clear Health Alliance; Sunshine Health: FL Medicaid & Serious Mental Illness; United Healthcare FL Medicaid & Veteran Affairs Community Care Network. Florida Medicare & Medicaid also accepted!

  • STEP 2 YOU request required REFERRAL from your physician {FAX 850.203.4429}. To optimize coverage, specify the reason for MNT and include supportive diagnoses, BMI, labs, and medical history. Medicare MNT coverage guidelines.

  • STEP 3 WE contact you once yourreferral PRIOR AUTHORIZATION is approved by your insurance company. GUARDIANS of the referred patient, please inform and provide validation.

  • STEP 4 YOU sign INFORMED CONSENT (DocuSign), as required to schedule an appointment and bill insurance (check spam) to treatment outlines service terms, telehealth, payment obligations, and release of PHI as required by HIPAA, and emphasizes your acknowledgement of privacy practices and jurisdiction. Our schedule is reserved for those who consent to treatment. *Advice is provided during appointments, and after your Informed Consent is signed. We do NOT prescribe drugs of any kind, nor injections for weight loss.

  • STEP 5‍ ‍WE SCHEDULE in Eastern Time Zone. All appointment confirmations come with a telehealth link via Jane. Best accessed on the Chrome browser. TELEHEALTH services by telephone, data-based video calls, fax, email, and text messages. A video consult is required for telehealth. *Respectful Appointment Etiquette: Please come fully dressed and camera-ready, on time, just as you would for a doctor's visit. To protect your privacy, ask your company to respect your appointment time and rules. **NUDITY** and/or **DRIVING** during appointments is **PROHIBITED** and (your session) will be discontinued until you respect your appointment time. HIPAA-compliant free SOFTWARE: Jane EHR -used for scheduling, appointment reminders, telehealth, notes/ documents, & billing; Spruceapp for encrypted communication; & DocuSign for Informed Consent.

  • STEP 6 ‍ ‍YOU accept Jane welcome prompt to set up a login and set your time zone. Please complete questionnaires before your appointment, and be prepared to take notes during our session. TO PAY, please add your credit card to your Jane App account. Your SpecialistCopay is due by appointment. For those without insurance coverage, Direct-Pay is $150 per hour, billable in 15-minute increments. We can guide you through a 10-minute process; additional time is billable and includes phone calls. *MEDICAID Members DO NOT have a Copay. NO SHOWS and/or cancellations with less than 24 hours' notice will be charged $50.

HEALTHCARE PRACTITIONERS

Complete REFERRALS are greatly appreciated, and required for Prior Authorization with the insurance company. {FAX 850.203.4429} ACCEPTS new patients of all ages, pediatric and adult! To optimize coverage, referral should specify the reason for Medical Nutrition Therapy MNT with supportive diagnoses beyond a BMI, and include labs, and medical history.
*SURGEONS are your authorizations denied due to BMI and barriers to nutritionist / dietitian LDN counseling? Along with your direction and patient care team, we can provide Statewide telehealth for your patients. Patients benefit from continuity of care, a warm handoff to an integrated team is evidence-based. Practitioners, case managers and office managers are welcome to email with questions. NO solicitations, please.
